IWB ... I don't get it.

Got a nice, thin kydex holster and a quality belt so it's not for lack of good equipment. Have carried a bit with a compact, single-stack 9mm. It's OK I guess but I spend all the time wrangling the damn thing into my jeans and getting the clip on then if it's sitting in the wrong place it's about imposible to adjust even an inch or so. And I'm not even fat. I have no idea how fat people can do it.

Once I've got it where I want it, I've still got to have a long enough shirt, or vest or jacket, that it doesn't expose the butt of the gun. And with a shirt / vest / jacket that long I might as well have a Desantis mini scabbard or something else OWB that rides up high and tight, and doesn't mess with how your pants fit.

I've never been comfortable with IWB, for whatever reason. Mostly use pocket carry and, once in a blue moon, an ankle holster.

I carry a FS 1911 in a Crossbreed at work all the time. "You don't mind, it don't matter".

I carry a full size 1911 in a Milt Sparks IWB Summer Special on a Beltman belt, all day, every day.
It does not bother me at all. But it might not work for everybody because...

IWB is pretty much the only way I carry, except for occasionally carrying my LCP in a pocket holster.
I never use kydex though.

I could never get IWB to work except with an LCR in appendix position, but I didn't carry there because I don't like the idea of appendix carry. Works for others, but not me.
How much bigger pants do you have to get to be comfortable?

I carry IWB most of the time. I prefer it behind my right hip near middle of back with gun butt facing left. I have a few cheap cordura holsters(gun mate) that work well after some modifications, but make most of my own.

There are many options out there with varying belt clip heights. The trick is to find the height that works for you. I like to have the gun butt just above the belt, and a 10-15* tilt doesn't hurt.

There are also "tuckable" models that can totally conceal the gun, but again, gun height is an issue. This is why I make most of mine, not very hard to do, just need to find a leather/harness shop to purchase leather(6-7oz), snaps, dye, nylon thread, wax, harness needles, etc. Make a pattern out of paper to your design, trace it on the leather and have at it.

The belt clips can be bought online if you do a search for them. Belt clips can also sometimes be found at LGS or gun shows in the used holster box for cheap. Bought a couple "junk" holsters the other day just for the clips, paid $1 each.

IWB holsters have their place but they're not for everyone.

I too carried a 1911 (Commander not full sized) in a milt Sparks IWB for years. It worked for me in that situation but everyone is different. I no longer use that combination but I changed my EDC to a different platform.

A few points:
An IWB holster MUST have a reinforced mouth so that the holster doesn't collapse when the gun is withdrawn. The inability to re-holster a handgun with one hand is a deal breaker and dangerous.

The key advantage of a IWB holster is its ability to conceal a long/thin gun with a shorter covering garment. That advantage decreases with a shorter/fatter gun and/or a longer covering garment. While a 1911 (long & thin) may work well in a IWB rig; a snubnose revolver (shorter/fatter) may not be a good candidate for a IWB rig.

Most IWB rigs require a tee shirt or something between you and the gun and this generally means two garments (tee shirt plus long tailed shirt) to comfortably carry the gun. This is not always the best combination in hot weather. Some IWB holster have a shielding piece of leather to keep the gun off of the body but even those are not really hot climate rigs.

IWB rigs are some of the most concealable rigs but often at the price of comfort.

It's a big toolbox, pick the right tool.

Never have even tried the IWB carry, although I've used the ankle holster for a small auto, as was previously mentioned, I don't like to try and re-holster an IWB. Myself, I now days I just belt carry under a jacket or a shirt untucked, or when it gets really hot, use a pocket holster. I don't like to wear bigger trousers than is necessary either.

Speaking for the fat people here I always carry IWB with full size guns. Good holster and you can't be a whinny butt. I do pocket carry my back up.

I never could get used to the IWB carry thing. I carry a full-size XD40 most times. For a while I used the OWB with the belt loops but quickly transitioned to the paddle version so it can be easily slid and adjusted to the perfect place for whatever position my body happens to be in, and it seems to cling closer to the body. Occasionally a shoulder holster might even be called for as well if riding the motorcycle or ATV.

Everyone is different though, and the most important thing is to find out what works for you. Most times it will take much trial and error and eventually accumulating a box full of discarded holsters.........

To me the keys are loose pants and stiff belt. I usually pocket carry but there are times I do IWB at 4:00. Doesn't bother me standing or sitting at that position. I also like the Supertucks because they have two belt clips and that put the weight over a wider belt area.
